S: Alexandra was seen at the Batt School for an additional rapport building session as well as a speech sound screening assessment. She was eager to work with the SLP throughout the session. Play-based therapy techniques and speech sound screening assessment were utilized to assist in developing an appropriate plan of care for Alexandra. Alexandra enjoyed playing with dinosaurs, unicorns, and magnetic tiles throughout the session. O: Through play-based therapy and speech sound screening assessment, a variety of speech sound difficulties were displayed. Errors patterns included incorrect pronunciation of /r/ and /v/ across all positions at the word level, deaffrication (e.g. a “stop” sound (/p/, /b/, /t/, /d/, /k/, /g/) replaces an affricate (/ch/, /j/), and depalatization/palatal fronting (/s/ for /sh/ and /z/ for /zh/). When the assessment was completed, stimulability for the /r/ sound was tracked at the word level via a play-based approach. Alexandra was stimulable in 10% of opportunities. A: Alexandra presents with speech sound difficulties that may impact her both socially and academically. The speech sound concerns mentioned above will be addressed in her updated POC. P: Begin language screening assessment and develop plan of care.
Introduce spelling words for the week and the meaning of the words. Work on using context clues to figure out the meaning. Introduce the passage for the week from page 108 - 111.
Show writing practice board and the levels on it. Show Libby how to write the letters of her name while using the names of the lines. Have Libby then write her name on the board. Read Mrs. Hen and Her Six Chicks. On the second reading, focus on pages one and two and have her practice sounding out the short vowel words.

We started a unit on the topic of the Environment. Today we broadly talked about the different aspects, such as water usage, meat consumption and cars. We focused particularly on the different roles cars play in Germany compared to the US as well as the availability of alternative means of transportation.

We completed the unit on personality traits and the comparison between being introvert and extrovert by talking and writing about our own evaluation, whether we have always been that way and whether we prefer being with introvert or extrovert people.
Work on how to use a diagram properly to show how things are different and alike. Use a T chart to list characteristics about horses and cows. Next show how to take that and place on a Venn diagram.
Introduce the concept of customs and relate to how we all do special things for each holiday every time we celebrate. Next describe things we do on regular days that are special to where we live in the US.

Lesson Outline
Practice using letter whiteboard to write her name with the letter placement. Practice nonsense words for short vowel sounds. Practice typing a few sentences while I spell the words and she finds and types the letters. Read a CVC story and practice sounding out the words to model it for Libby.
