Luke started off the session well today. He listened to two listening comprehension stories that were grade appropriate. He scorse 4/5 and 2/5. Then he worked on the L sound in the initial position. He did two different L worksheets to work on production. 80% accuracy.'Look at the ___' with various L words. 100% accuracy.
Richard started off the session describing his weekend activities. Then he listened to grade appropriate listening comprehension stories. He scored 4/5 on the first, 3/5 on the second. Then Richard did a compare and contrast activity. For this he was read two short paragraphs and asked to pay attention to what made the stories similar and different. He answered 6/8 questions correctly. Then he listened to two sequencing stories. These were tricky! He was able to put 3/4 of the events in the correct order in both stories.
The session commenced with Chris excitedly awaiting feedback that he was hoping to receive regarding his latest video production. He happily accepted the positive feedback and detailed description regarding the story line, use of cohesive ties, temporal links and elements of story grammar that he had included. Conversational reciprocity was strong this session as contingent comments were provided with increased frequency. It is significant to note that use of increased novel language and expanded vocabulary continues to increase. Attention and motivation continue to remain strong each session. Perspective taking continues to improve as well. Target /s/ phoneme production was noted with increased lingual retraction at the single word level with 90% accuracy. Strong session once again.
Richard had a great session today. He helped a classmate problem solve a task by following directions. Then Richard worked on 6-step sequencing pictures. He is working on being concise and to the point when re-telling a story. Richard did 4/4 with 100% accuracy. Richard then listened to short problem solving scenario and was asked to state logical conclusions. 3/4 with minimal cues.
S: Noah was seen at the Batt School for treatment. He was eager to work with the SLP and compliant for the duration of his treatment session. Play-based, child led therapy, along with structured language tasks utilized. Re-evaluation needs discussed with Noah’s dad at end of session. O: Noah completed an alphabet identification activity in which he had to identify the letter provided verbally given initial verbal/visual model in a field of 3. He completed the activity with 60% accuracy. Noah then completed a following directions activity where he was asked to follow various 2-step directions. He completed the activity with 25% accuracy. Noah also followed directions utilizing a variety of spatial concepts (e.g. on, under, next to). He completed the activity with 60% accuracy. Then, Noah completed a spatial concept identification activity where he identified the spatial concept word provided in a field of 3 pictures. He completed the activity with 60% accuracy. Lastly, Noah completed a speech sound activity with /l/ and /r/ blends at increasing levels (i.e. word, phrase, sentence). He completed the task with 68% accuracy when given an initial model. A: Noah displays some increase with letter identification. Difficulty with completing both steps provided in 2-step directions noted, often completing the first step only. Continued difficulties with /l/ blends noted. P: Continue with current plan of care. Begin assessment process during next session.

Luke then worked on some personal story sequencing, 85%.
Luke worked on L sound production- he was a little silly at the start and stated 'I call myself WUKE sometimes' and then shifted and was able to produce L correctly in the initial and medial position with 90% accuracy. He was able to hang onto the L sound in a simple conversation with 90% accuracy. Great focus and attention today.

As the session commenced Chris initiated conversation with an "announcement." He laughingly shared that he "had a lot of important stuff to talk about today!" Initially, Chris was eager to discuss the status of his move productions that were currently "in the works." His use of detail, and expanded vocabulary, was strong this session. Speaker listener reciprocity was characterized once more by increased cycles of exchange. Use of contingent comment across 4/6 conversational turns 8 times was noted within the session. Narrative generation was characterized by increased use of story grammar elements. In addition, use familiar scaffolding strategies facilitated increased linguistic organization. Attention and motivation continue to be strong, with a noted increase in self awareness, presupposition, and mindfulness of partner's body language, gesture and intonation.

Luke had a great session today. We started out with working on L in conversation and doing a 'fall' leaves activity. Luke was able to produce L in the initial, medial, and in blends with 85% accuracy with very few reminders. Luke then sequenced a 4 part story using sequencing terms with 100% accuracy. Lastly, Luke listened to a small story (heavy on L) and answered questions about the story. 4/6 questions correct. He did an excellent job pronouncing his L sound correctly when answering the questions.
Richard started the session off by listening to sentences that increased in complexity. He was given a word to provide a synonym for. Richard was able to give an accurate word for 26/35 synonyms.
Richard then put together 3 6- step sequencing stories. Richard was able to verbally sequence the stories correctly using first, second, and third terms. Richard was 100% accurate.
Next Richard did an excellent job identifying problems in pictures and coming up with solutions. He continues to have some difficulties with tenses when telling stories.
The session commenced with Chris rapidly sharing that some of the "ideas" that he had shared last week have changed. He was eager to clarify the details, time line sequences, characters and release dates that he modified in both his upcoming plush video productions as well as his comic book series. This afforded an excellent opportunity to expand upon aspects of conversational exchange and presupposition. Speaker- listener reciprocity continues to expand, characterized by increased cycles of exchange. Quality of exchanges continues to include strong use of affirmation, however, expanded use of contingent comment is now noted with increased frequency. Topic transition continues to improve as well, with spontaneous use of transition phrases noted more frequently. The decreased need for scaffolding and maximal cue supports is now noted on a session to session basis.
