S: Alexandra was seen at the Batt School for therapeutic intervention for both a language and speech-sound disorder. She was eager to play/work with SLP throughout the session. Play-based therapy techniques and structured language tasks were utilized throughout the session to target POC goals. O: Alexandra began with speech sound activities in which she produced /r/ blends and /sh/ in the medial position of words when provided with an initial model. She completed the activities with 90% and 45% accuracy respectively. She then answered basic “wh” questions given a field of 3 visual/verbal answer choices. She completed the activity with 65% accuracy. Last, Alexandra completed an expressive spatial concepts activity in which she was asked to identify the spatial concept provided given a visual/picture (e.g. a picture of a cow behind a barn with the prompt, “Where is the cow?”) and a field of 3 answer choices. She completed the activity with 70% accuracy. A: Alexandra displays progress/stability on POC goals. P: Continue POC.
This morning, Alexandra and I incorporated math activities into active play outside. We used 15 hula hoops to jump through and count, colored cones to match to their corresponding hula hoop, and both were used to make patterns. Then, once inside, Alexandra played number recognition Bingo with the numbers 1 through 50.
Alexandra was so engrossed in putting together floor puzzles that we didn't get to complete the project. She asked for assistance with one of the puzzles so I helped her find all the pieces with a flat edge. Then, she was able to put it together. After that, we went to the preschool and she designed a place for Splashy, the dolphin, and Swimmy, the turtle. Lastly, we visited the chicks for one last time.

Today, Alexandra worked on a counting activity. She counted the number of frogs on logs, wrote the amounts, and colored the pictures. Then, we worked on the concept of taking away with subtraction picture problems. Alexandra counted and colored groups of fish, crossed out the fish swimming away, and wrote the number of fish left.

Alexandra completed her interactive calendar, read her sight words, finished her calendar talk and calendar writing. put together her daily sentence puzzle, worked on her phonics activity, watched a video about the lungs, read and answered questions about the lungs, and made a working 3d model of the lungs.
Today's focus was on counting to 100 and understanding patterns on the hundreds chart. Alexandra and I counted to 100 by 1s and 10s and looked at row and column patterns on the chart. Then, we solved addition within 10 problems using pictures and finished with a dinosaur puzzle.
This morning, Alexandra and I worked on number bonds and finding the missing addends with sums to 5 and 10. Alexandra found the missing parts of equations using her fingers and a number line to count forward from the first addend to the sums of 5 or 10, determining the missing part.
