Alexandra completed a speech therapy screening/evaluation today. A write up is being done with her errors and goals. At this time Alexandra demonstrates errors on 9 sounds. Will target K, G, L and V to start.
Today we completed CELF 5 testing, all 9 subtests. Will score test, write evaluation, and schedule time to discuss results with parent. Luke did an excellent job sitting during the evaluation today and completing the final 2 subtests.
I wrote an assessment report based on the informal reading inventory, Orton Gillingham phonics checklist, and writing assessment that I conducted with Cade.
S: Noah was seen at the Batt School for a re-evaluation of speech sounds to assist with a plan of care update. Noah was eager to work with the SLP and compliant for the duration of his session. Frequent breaks utilized to gain/maintain attention while administering the Goldman Fristoe Test of Articulation, Third Edition (GFTA-3). O: Noah completed the GFTA-3 (both sounds-in-words and sound-in sentences portions). After completing test administration, Noah completed a spatial concepts activity in which he identified the spatial concept in a field of 3 pictures with 50% accuracy. A: Noah benefitted from multiple verbal prompts/repetitions during sounds-in-sentences portion of GFTA-3. Results of this section should be interpreted with caution due to difficulty following directives for the section. Limited data noted on spatial concepts activity. P: Continue evaluation process with administration of the Preschool Language Scales, Fifth Edition. Upon completion, full evaluation results will be provided and explained to parents. Plan of care will also be updated based on results.
Richard did an excellent job attending, focusing, and sitting for a speech-language evaluation today. He completed 6/7 sections of the test and asked to please do the final section during his next sesssion.
Child seen at school for initial session for an occupational therapy evaluation. Child was given the Beery VMI, sixth edition. The VMI consists of three subtests which evaluate a child's visual perception, motor coordination and visual motor skills. We also used the Brunink's-Oseretsky Test of Motor Proficiency, Second Edition. This evaluation assesses several areas of a child motor control with the subtest of: fine motor precision, fine motor integration, manual dexterity, upper-limb coordination, and balance. This test will continue next treatment session. Please refer to written evaluation for details once evaluation process in complete.
Physical therapy initial evaluation was completed today. Richard was very energetic and performed all activities asked of him. Richard demonstrated excellent mobility and functional play activities. He demonstrated difficulties with core, hip and foot strength in all standing and functional activities. He had difficulty maintaining his hips and feet in the neutral position during static standing activities, however he tolerated neutral position well with assistance. He prefers to walk on his toes with all walking and running activities. Richard demonstrates excellent potential to increase core strength, perform heel toe gait pattern, and increase dynamic coordination activities. Thank you for allowing me to work with him;)
