Libby seen in her room for a 45 minute occupational therapy treatment session. Focus of therapy on improving her ocular motor skills, visual perceptual abilities, fine motor development, visual motor skills, sensory motor skills and graphomotor skills. Began treatment with executive functioning exercises with identification and recall. Libby is now able to recognize 100% of the uppercase alphabet without pause. We worked on fine motor refinement while Libby colored wooden 3-inch letters using small distal movements. Today we learned how to form the lowercase letters "w and y". Libby was taught the specific line placement of these letters with adequate understanding. She has been forming the letter "y" from right to left versus left to right, making the "long stick" first. She was instructed today that "y" is formed by making a "short stick" then a "long stick". Good focus, good session.

Libby seen in her room for a 45 minute occupational therapy treatment session. Focus of therapy on improving her ocular motor skills, visual perceptual abilities, fine motor development, visual motor skills, sensory motor skills and graphomotor skills. Began treatment session ocular motor tracking activity. Libby was required to find her way through 1/2 inch wide maze. She was observed to hit the sides or "go off the path" frequently but was able to solve the maze quickly. Continue with another maze in which Libby had to get through by following arrows which pointed up, down, left or right. She required max. assistance to make her way through this type of maze. She enjoys doing mazes, even if difficult, and asks to do more. Lastly, we reviewed previously learned letters in lowercase print, concepts of "leaf" letter (tall lowercase letter), "trunk" letter (sit in the middle of the writing line), and "root" letter (goes under the writing line). Libby has not completely grasped this concept yet. She was also instructed on the lowercase "u" today and wrote it well. Have a wonderful Spring Break.

Libby was seen for a 45 minute treatment session in her room for occupational therapy services this week. Focus of session continues to be on improving her fine motor skills, visual motor skills, sensorimotor processing/registration skills, ocular motor development and self-help abilities. Today's sessions began with hand strengthening exercises to increase her endurance for writing using a tripod grasp pattern versus thumb wrap grasp. Continue with Print Island practice. Today's session focused on learning the concept of lower case letter sizing and where letters are placed on the writing line. Libby was instructed using the terminology from Print Island. Tall letters are called "leaf letters" (b, d, f, h, k, l , t) some letters stay in the middle of the writing line and they are called "trunk letters (c, o, a, e, i, m, n, r, s u, v, w ,x and z), and lastly she learned the letters that go under the writing line are called "root letters" (g, j, p, q and y). OT will continue to reinforce line placement, sizing and formations of the lower case letters. Good session.

Libby was seen in her room for a 45-minute treatment session with the focus of therapy on fine motor skills specifically grasp patterns, ocular motor skills (saccades), self care skills, and handwriting. OT has also focused on sensory processing and having Libby be more comfortable with wearing a variety of clothes (weather appropriate) and different hairstyles (only wants to wear her hair one way which is down). Began session warming up her hands with discovery putty to find hidden objects all having to do with a farm. Libby had her hair down and OT asked if she could put it up in a barrette. Libby agreed without hesitation. Continued working on instruction on lowercase alphabet. Libby is more than halfway through with instruction and is making steady process with visual recall for the lowercase letters. Today she was instructed on the the "fall down jump up letters" - j and k. She was instructed that "j" is a root letter and goes under the writing line and that "k" is a tall leaf letter and starts by the top line. She need assistance in making "k" in a fall down, jump up fashion. She has a incorrect motor memory for how it is made. By the end of the session and with verbal cues, demo, and visual cues, she was able to make a "k" adequately. We will continue to practice this in therapy sessions.

Continue to write in lowercase when working with focus on proper letter formations.

Libby was seen in her room for a 45-minute treatment session with the focus of therapy on fine motor skills specifically grasp patterns, ocular motor skills (saccades), self care skills, and handwriting. OT has also focused on sensory processing and having Libby be more comfortable with wearing a variety of clothes (weather appropriate) and different hairstyles (only wants to wear her hair one way which is down). Upon OT's arrival Libby was wearing her hair in a barrette. It was falling out a little bit and she let OT fix it without complaint. Began treatment with ocular motor task to work on tracking and saccades. Libby had to find her way through a maze with moderate difficulty. She required moderate assistance but demonstrated increased task endurance and persistence. Worked on kinesthetic awareness of letters. OT had Libby close her eyes while she took her hand and drew the previously learned letters on sandpaper to see if she could identify the letter based on the movement used to form it. Libby was able to recall the letters "m, c, o, g, s, f and "r" based on movement alone. She required the movement to form the letter and auditory cues to recall and identify the letters "b, a, q, n, and d". With vision occluded confused the lowercase "p" with the letter "d". Today she was instructed using Print Island on a new group of letters called "Fall Down, Jump Up" letters. She was instructed on the lowercase letters "l, i, and t" which she formed well. Great session, Libby worked hard.

Libby should have something to stabilize her feet because her desk and chair are so high. For writing it is better if she is not pivoting in a chair that spins and her feet are dangling off the ground. Really her desk and chair are way to high for her and should be smaller and lower.

Libby was seen in her room for a 45-minute treatment session after two week winter break. Libby was happy and worked well during today's sessions. The focus of therapy on fine motor skills specifically grasp patterns, ocular motor skills (saccades), self care skills, and handwriting. OT has also focused on sensory processing and having Libby be more comfortable with wearing a variety of clothes (weather appropriate) and different hairstyles (only wants to wear her hair one way which is down). Began treatment session with fine motor exercises with putty and finding hidden toys/little objects to increase hand strength. Continued with visual discrimination and tracking tasks to work on ocular motor skills via mazes and "I Spy" book. Libby enjoyed doing mazes - with minimum to moderate difficulty. When maze got difficult and she had trouble figuring out the way she wanted to "quit". OT helped her stick with it and get through maze before going to next one. Libby needs encouragement when things have increased difficult but with encouragement she is able to be successful. We continue to work on handwriting and reviewing/practicing letters she has already learned. Today we focused on the "c" first letter group and specifically the letters "d, g and q". Libby continues to need max. assistance to write the lower letter "d" with a "c" first. She practiced "g and q" and wrote them adequately; however, they need more practice. Libby worked well today!

Libby was seen in her room for a 45-minute treatment session after two week winter break. Libby was happy and worked well during today's sessions. The focus of therapy on fine motor skills specifically grasp patterns, ocular motor skills (saccades), self care skills, and handwriting. OT has also focused on sensory processing and having Libby be more comfortable with wearing a variety of clothes (weather appropriate) and different hairstyles (only wants to wear her hair one way which is down). Began Libbys session with hand strengthening activities to create a more stable base for fine motor tasks. Libby does well with different exercises/games to increase her hand strength. We want to build the strength of the intrinsic muscles in her hands and endurance for writing and other fine motor tasks. We continue to work on learning handwriting through Print Island. When provided with visual and auditory cues Libby is able to write approximately 50% of the lower case alphabet well. She is being taught these lower case letters using a multi-sensory approach which encompasses auditory, visual tactile and kinesthetic cues. She learn well with this approach. She continues to make gains with letter recognition of both upper case and lower case letters. Progress has been steady this school year and she continues to be a pleasure to work with.

Libby was seen in her room for a 45-minute treatment session with the focus of therapy on fine motor skills specifically grasp patterns, ocular motor skills (saccades), self care skills, and handwriting. OT has also focused on sensory processing and having Libby be more comfortable with wearing a variety of clothes (weather appropriate) and different hairstyles (only wants to wear her hair one way which is down). Libby was happy upon therapists' arrival. She told OT that she moved into her new house and loved it. Began treatment session with writing and creating on a "Buddha Board". On this board Libby used a paint brush with water to write letters. She was excited to tell the OT that she could write some three letter words and wrote the following words in uppercase CAT, PAT and MAT. During todays session we reviewed the lowercase letters c, o and a. Libby was instructed on correct formation of the lowercase letters d and g today. She required hand over hand assistance at first to make the "d" with a "c" first. She required visual cues and visual boundaries to form the "g" correctly as well. She needs continued practice to form these letters correctly. Continued gamer with site word fishing game. She had difficulty with reading the site words so we practiced her recall for lowercase letters and she would read out the letters of the words if she was unable to read the word. Good session.

Libby was seen in her room for a 45-minute treatment session with the focus of therapy on fine motor skills specifically grasp patterns, ocular motor skills (saccades), self care skills, and handwriting. Libby was happy today and participated well during her 45 minute session. Began treatment session with resistive putty to work on intrinsic hand strength. Libby is observed to use a immature grasp pattern on her writing utensils at times and by increasing her hand strength she was have more control and stability for fine motor tasks. Worked on visual motor development and executive functioning skills by going on previously learned print lower case letters. Libby does best when provided with some verbal and visual cues to recall form of some of the lower case letters she has previously learned. She review the first group of letters learned which included "r,n,m,p,f,b, and h" and worked on "c,o and a" today. Good session.
