S: Noah was seen at the Batt School for treatment. Noah had some difficulty with transitioning to session, but he was eager to work with the SLP for the duration of the session. Evaluation results and updated plan of care was given to Michael, Noah’s babysitter, as well as put on file at the Batt School. O: New plan of care goals were probed and initial baseline data taken. Noah was asked to follow a variety of 1-2 step related directions. He completed the activity with 20% accuracy. He was then given a picture scene and asked mixed “wh” questions about the picture. He answered the questions with 25% accuracy. After, Noah was asked a variety of “wh” questions about a short passage read to him, given a field of 3 visual answer choices. He completed the activity with 15% accuracy. Lastly, Noah asked to identify quantitative and spatial concepts, in fields of 3 visual answer choices and 2 visual answer choices respectively. He completed the activities with 25% accuracy and 20% accuracy respectively. A: Noah benefitted from increased prompting throughout tasks. Initial baseline data collected. P: Continue collecting baseline data on updated POC goals.
