S: Noah was seen at the Batt School for treatment. Noah was eager to work with SLP for majority of the session. Some task avoidance noted when completing some activities. Reduced session time noted. O: Noah first identified the letters in his name. He completed the activity with 100% accuracy. He then participated in an alphabet identification task, identifying all letters of the alphabet in a field of 3 letter choices. Noah identified the letters provided with 81% accuracy. Next, Noah was asked to identify the superlative (e.g. big, bigger, biggest) in a field of 3 visual answer choices. He completed the activity with 80% accuracy. Lastly, Noah completed a letter-sound correspondence activity in which he listened to the letter sound and identified the letter in a field of 3 answer choices. He completed the activity with 70% accuracy A: Noah displays consistency/increases across all goals. Task avoidance noted on letter-sound correspondence activity. P: Continue POC. Monitor/discontinue superlative goal.
