S: Noah was seen at the Batt School for treatment. He was eager to work with the SLP and compliant for the duration of his treatment session. Play-based, child led therapy, along with structured language tasks utilized. O: Noah completed an alphabet identification activity in which he had to match the letter provided to the correct letter given initial verbal/visual model in a field of 3. He completed the activity with 50% accuracy. Noah then completed a following directions activity where he was asked to follow various 1-2 step directions. He completed the activity with 55% accuracy. Noah also answered mixed “wh” questions immediately following visual/verbal info given a field of 3 answer choices with visuals. He completed the activity with 60% accuracy. Then, Noah completed a speech sound activity with /l/ and /r/ blends at increasing levels (i.e. word, phrase, sentence). He completed the task with 65% accuracy when given initial model. Lastly, Noah’s play skills were observed with data collected during a variety of play activities. He displayed the ability to turn-take/share in 70% of trials. A: Noah displays some increase with letter identification. Increased level of distractibility noted during “wh” question task. Continued difficulties with /l/ blends noted. P: Continue with current plan of care.

Richard had a great start to the session today. He sequenced a 6-part story with no help. Richard then verbally arranged the story and made it into a story using sequencing terms with minimal difficulties. He had some difficulties with past tense but when given the chance to change his tenses, he did! He got 6/6 right on the second sequencing story. We will now be working on staying in the same tense. This was difficult as he went back and forth with present progressive and the past tense.
Next Richard participated in an irregular past tense verb activity. He was read a sentence that had a present tense verb and asked to fix it. 8/10
Next Richard was read a short 10 paragraph story outloud and asked questions about the details and to fill in the blanks. He got 7/7 of the questions correct after listening to the story. He was very focused!
Lastly, Richard was read 15 sentences that had a vocabulary word for him to come up with a synonym for- this was challenging, Richard came up with 8/15 synonyms. He shared that he did not know many of the words but was using the sentence to help determine the meaning.

Chris was eager to "talk about a lot of stuff" this morning. He utilized familiar topic initiation strategies, and use of topic transition phrases was strong as well. Prompts to successfully organize details and utilize increased vocabulary variation were successful. Use of "practiced/previously learned phrases" was greatly diminished this session. Request for clarification when "learned phrases" were used, was met with both successful explanation, as well as instances of uncertainty. Allowance for expanded conversation regarding the "catch phrase or phrases " utilized when admitted uncertainty was noted was strong. Use of cohesive ties within expanded narration was supported successfully. Speaker -listener reciprocity was strong once again today with notable exchanges consisting of 4-6 cycles per subtopic. Response to clinician initiated topics was characterized by use of affirmation, and increased comment. Motivation continues to be high and attention was strong this session. Target /s/ phoneme production is now noted with increased lingual retraction at the single world level 85% accuracy. Strong session today.

Luke did a nice job today with 3 step story sequencing. He used first, then and last correctly 75% of the time with minimal cues. He had some difficulties with coming up with solutions to problems during a problem solving activity. Lastly, we worked on L in the initial and medial positions. He did great! Initial L 80% accurate with minimal cues and medial L 60% accurate with minimal cues. He got out a picture book and used L correctly to describe a picture with no cues. Trials with L at the end of the session- 95% accurate in initial and medial. He tends to joke around and intentionally do a W but at the end he did an excellent job focusing and getting his tongue in the right position. Great carryover. Sending home two L worksheets in his orange folder in backpack for home practice. 2 minutes per day.

Luke was excited today. He shared that his birthday is next week.We started the session working on the L position. Luke used a mirror. He did a nice job finding the position. He was able to produce L in 50% of opportunities correctly today in the initial position. Next Luke worked on what questions. 80% accuracy with no cues.Lastly, Luke worked on sequencing 3 picture sequencing stories. He did a great job. 5/7 with minimal cues given.

Started the beginning of the session by sitting down with Richard's mother and discussed his positivbe Evaluation results. Discussed goals as well.

Today, Richard worked on sequencing 3 step stories using first, next and last 100% accuracy.

Richard listened to reading comprehension stories and answered questions about the main idea(100%), details (80%) and sequence (50%).

Noah then completed a spatial concepts activity where he reviewed spatial concepts (e.g. in, on, under, behind) and was asked to follow directions using the spatial concepts. He completed the activity with 65% accuracy. Noah completed a sight word matching activity in which he was asked to match the sight word provided to the correct sight word in a field of 3 answer choices. He completed the activity with 20% accuracy. Lastly, Noah’s play skills were observed with data collected during a variety of play activities. He displayed the ability to turn-take/share in 70% of trials. A: Noah displays consistency across most POC goals. Decrease displayed on sight word goal likely due to distractibility during task. P: Continue with current plan of care.

Richard had an excellent session today.
He worked on sentence assembly at that start of the session. This was difficult due to his difficulties reading but he did a great job trying to remember the words on the picture cards in order to form complete grammatically correct sentences.
Richard worked on TH in the initial, medial and final positions of words at the word and sentence level. He did so with 90% accuracy. In conversation, he is about 60% accurate with no cues.
Lastly, Richard worked on paragraph comprehension. On the first paragraph he got 5/5 questions correct. On the second paragraph, which was very long, Richard got 6/6 questions correct. He did an excellent job sitting and attending to the paragraphs read aloud to him.

Chris continues to eagerly initiate each session with enthusiasm. He is consistently prompt and well prepared. This morning's session commenced with Chris sharing that he had "lots of stuff to talk about today." He was excited to share the new ideas and changes that he had in mind for his upcoming productions. This centralized topic continues to provide a springboard for enhancing conversational reciprocity and supporting effective topic transition. Use of familiar "transition phrases" continues to increase in frequency. Speaker -listener reciprocity was strong today, expanding to 4-6 cycles per subtopic, with diminished speaker -speaker overlap noted once again. Instances of overlap were identified more readily, and self correction/repair attempts were successful 4 times during the session. Attention was sustained for the duration of the session once more. Strong session.

Richard started the session off working on paragraph/story comprehension. He read the story 'The Closet Closure' and then answered 2.5/5 questions correctly about the story. He had a difficult time coming up with adjectives to describe the main character in the story and difficulties with the sequencing of the story. He did a nice job being able to tell who the main characters were, the setting of the story, the problem and the solution of the story. Next Richard listened to a poem called 'Raccoon Rex' and then was asked 5 follow up questions. He answered 4/5 correctly. He did not know what 'bounty' meant in one of the questions but was able to come up with a synonym for the word 'stealth' on this own. Then he did a vocabulary match with synonyms that was difficult and required max prompting. Lastly, he listened to a story called 'Where Does Chocolate Come From?' On this story, Richard did a nice job recalling specific details.

TH- Richard worked on initial, medial and final TH in sentences today with 80% accuracy.
