Noah transitioned very well into the session today. He requested magna tiles. During magna tiles we worked on conversation. A peer his age was on the playground and Noah asked to play. We went outside. Noah answered 'What is your name?' and then with prompting asked the peer 'What is your name?' Noah did an excellent job engaging with this peer. Noah wanted the peer to come play on the slide with him but did not know how to ask. With prompting, 'Do you want to come on slide?' Great social interaction/language opportunity.
When back inside, Noah focused on the present progressive form to increase sentence length. Noah looked at a variety of action pictures and was able to give 3+ words to describe them. He is not yet independently using he/she/girl/boy/they to describe the pictures but rather says 'eating a hot dog.' We focused on adding a pronoun/noun to the start of the sentence. With cues, he was able to do so 75% of the time.

Noah happily transitioned into the session today. He was excited to get started and immediately requested the vet backpack. After working on language skills with pretend play (requesting, commenting, etc) we moved onto articulation. Today the focus was on L-blends. We worked on
- black bear
-blackberry
-flower
-climb
-glasses
-glue
-plate

With cues, Noah was able to produce L correctly in all of the blends. Without a cue, he continued to produce a /w/ for the L sound. "Cwimb" instead of "climb." He required moderate cues, 50% accuracy.

Noah requested play doh at the end where we focused on the L in play doh. He was able to say play with the L 5x.

Noah was upset during the transition but settled down after about 1 minute. He showed the clinician his Christmas card and named all of his family members. He was very proud. Multi syllabic words in short sentences focusing on L sound- 75% accuracy. Sending home L blends (claw, blanket, flute, gloomy, play, and slug) because L blends seem to be the most challenging.

Noah is doing a significantly better job communicating his wants and needs using sentences. He is using 5-6 words together when he requests, protests and comments. He is also starting to be able to go back and forth with some beginning conversational skills.

We finished with WHERE questions- he did not need a visual cue on 5 of them today which is a huge improvement. He was 75% accurate.
Overall significant improvement with articulation and language skills today.

Noah transitioned very well into the session today. After not seeing him for a week, I was very impressed! Ms. Judie got a present for Noah today which was a great language learning experience. Noah took the vet/dr bag and did a 'check up' on Ms. Judie today as well as Pluto. We worked on him vocalizing 'can I check you?' 'Im going to look at your ____ now.' Noah became pretty shy doing check ups but it was great practice for his language skills and practicing answering questions.

Noah focused on answering and asking questions. We did where questions today 6/10. He requires a prompt to answer 'You wear socks on your ____.' when asked where do you wear socks.

Overall I want to to continue focusing on asking/answering questions especially with other people! It is a great way to encourage increasing sentence length and working on articulation. No jargon/nonsense speech noted today.

Noah transitioned well into the session today. No tears and came and hugged the clinician! He was excited and ready to work. First he worked on what questions with pictures and a choice of 3. Much improvement noted. He has started answering in 3-4 word phrases. 'A coat when raining' when asked 'what do you wear when it's cold?'When asked 'what opens a door?' Noah replied 'Open the door with key.' Noah transitioned to some pretend play with the doctor kit. He is doing a better job of explaining and communicating what he wants to do. 'I going to check.' 'I cut a finger.'
Next we focused on multi syllabic words. When Noah is focused and taps out the syllables, he is 90% accurate.
Overall his articulation seems to be improving significantly. Very little jargon or nonsense words are heard during sessions.
Noah found a pretend phone and said he was talking to his dad. He said 'Ok ok I call you back' and 'I got my phone'
Noah also did some pretend play with the kitchen. He had difficulties answering questions during this play like 'what are you cooking?' 'Who is dinner for?' etc. Continue to practice pretend play while asking open ended questions for him to answer. Can also work on choice of two..are you cooking spaghetti or chicken? This still allows him new vocabulary and to answer in a phrase.

Noah transitioned to today's session better than Tuesdays. He calmed down much quicker and engaged with the clinician quickly! He was excited to play with the trains. We worked on requesting each item out of the box with clear articulation. While playing with the trains, Noah drove the train to multi syllabic word cards (dinosaur, helicopter, lollipop) where he had to pronounce the word before moving beyond the card. Noah did a great job with this and had fun. Noah then worked on the present progressive form where he produced short sentences/phrases 'He/She is ____' Today we added some places and objects at the end of the sentence to increase sentence length. Overall, today it was noted that he is using more words per sentence consistently. 'I can't find the doctor.' 'I looking doctor' 'I am going on rocket ship with daddy and mommy'-

Noah had a difficult time transitioning into today's session. He used excellent language skills to protest and request what he wanted. After 10 minutes, Noah was settled and ready to play with the clinician. After playing with the trains for a few minutes and using good language skills to communicate and request, he transitioned nicely to a book where we discussed what, who, and where questions.
Wh questions- where 90% accuracy, who 70% accuracy, what 80% accuracy.
Noah reached for the dr. kit again today. He really enjoys pretend/role playing with the dr. set. Today we focused on asking 'can I check you?' rather than just checking. We worked on saying phrases like 'Open your mouth' 'Sit still' 'Take a breath'
Noah required a little more breaks today but overall had a good session. We finished the session with action picture cards 'He/She is ___' 5/10 action pictures with pronoun+ verb+ing -

Noah transitioned easily into the session today. He was ready to work! We started off with multi syllabic words to help increase overall intelligibility. Noah did a great job with 2-3 syllable words. Some 3-4 syllable words were difficult such as elephant. We focused on slowing down and tapping out syllables to help cue him. He is ~75% accurate. Noah engaged in a good amount of pretend and imaginative play today. He enjoyed playing doctor and role playing. He worked on using present progressive form while naming action picture cards. He did a nice job naming the action but had difficulties putting it in sentences. Sent home action picture cards to continue working on putting actions in full sentences. 'What is she/he doing?' and then Noah replying 'He is____' or 'She is ___'

Noah transitioned very well into the session today. He greeted the clinician and said 'I go play toys!' We started working on WHERE question cards and Noah had fleeting attention. During this activity, Noah said 'tummy tummy' and I asked if his tummy hurt. Noah said 'Tummy tummy hurts' and a minute later needed to use the restroom. He is doing a nice job of verbalizing these requests clearly. He continues to add in jargon/non sense in between real words/phrases.
Noah was about 50-60% accurate with where questions. He benefitted most from a choice of two.

Noah worked a lot on pretend play today and expanding utterances. He did excellent play with the people from the toy house. He put them to sleep, made them food etc. He used the following phrases:
" I sweeping'
'I pee pee in the toliet at noah school'
'I can't open it'
'I cooking now'

During most of this play today, I worked on sentence closures with him. He would say 'I cooking' and I would say 'I am cooking _____' and wait for him to fill in. He did not always fill in but it is a good tool to get him to use more words per utterance.
Lastly we worked on multi syllabic words- 80% accuracy with minimal cues.

Noah had a much better session transition today. He cried for a few minutes but enjoyed walking to the classroom. He warmed up much more quickly today and worked hard. Worked on Wh question worksheet with some visuals. These were difficult. What questions 2/10 with no cues. His accuracy increased when provided a choice of two. Do you wear a hat or a shoe on your head?' 'Do you hear with your nose or your ear?' Continue to give visuals and/or a choice of two items for what, where, and when questions.
Noah did a nice job requesting to use the bathroom during today's session independently.
Continue working on pronouns. Focusing on using I instead of 'Noah' when referring to himself.
L sound in conversation and direct practice: 70% accurate with minimal cues.
Object function choice of 3 with visual cues, where questions and combination questions sent home for more practice.
