Noah initially transitioned well into the session. About 5 minutes in, he had a difficult time. During his difficult time, the clinician asked him to use the restroom. He did not make it to the bathroom and had an accident. He recovered well and had his best session yet! He was focused and engaged very well with the clinician. He worked on multi syllabic words (2,3, and 4) in phrases and sentences. He also worked on action picture cards using the present progressive form in short sentences. Wh - questions were the most challenging as it appeared that he had difficulties on what type of answer was expected with a who or what. Noah did a nice job with eye contact and engaging in some attempts at conversational speech. He answered questions about halloween. At the end, during free play with the clinician, he used 3-4 word phrases consistently. 'I like the trains,' 'I going to sit now' 'I need money' 'I all done now' and 'Clean up trains' -

Noah had a difficult time transitioning into the session today. After about 5 minutes, he was warmed up and ready. He requested to play with pop the pig. Worked on complete and clear sentences. Then he requested magna-tiles. Worked on WH-questions-
Where do you sleep? 'I sleep in a bed noah'
Where do you sit? 'I sit at a table'
Where do fish live? 'In the boats'
/l/- 60% accuracy in single words, with mod cues, able to produce /l/ sound correctly in words-
Some of the wh-question responses were unintelligible. Continue to work on articulation and wh questions next week.

Noah had an excellent session today. He took a few minutes to transition into the session but participated well. Noah worked on action picture cards ( verb + ing). Noah did a nice job of using the present progressive -ING but required cues for pronoun+ is. Noah also worked on multi syllabic words for increasing his intelligibility. He did a great job, 80% accuracy.

Today was Noah's first session. PLS-5 was administered to examine his expressive and receptive language skills. The GFTA was administered to examine his articulation skills. A report will be provided to Noah's family with observations, scores, and goals.
