S: Noah was seen at the Batt School for treatment. He was eager to work with SLP for most of the session. Some task avoidance noted. O: Noah was first asked to sort items provided into different category spaces of where one would find them (e.g. school, park, grocery store, and clothing store are the places, items to be sorted could include a backpack, a swing, a steak, a jacket, etc.). He completed the activity with 64% accuracy. He was then asked to identify which item, given a field of 3 picture choices and two adjective descriptors, was being described. He completed the activity with 58% accuracy. Noah then completed a pronoun labeling activity with moderate support and verbal answer choices which he completed with 50% accuracy. After, Noah was asked to identify the superlative (e.g. big, bigger, biggest) in a field of 3 visual answer choices. He completed the activity with 60% accuracy. Noah then completed an alphabet identification activity where he found the uppercase letter provided verbally in a field of 3 letters. He completed the activity with 54% accuracy. Lastly, Noah was asked to match the sight word provided given a field of 3 sight word answer choices. He completed the activity with 100% accuracy given 5 trials. A: Increases/consistency displayed across most goals. Minor decreases displayed on some POC goals likely due to task avoidance. Probes for literacy goals started. Some basic sight word exposure incorporated within session. P: Continue POC. Continue to develop/probe literacy goals.

The session commenced with Chris sharing thoughts as to why he removed many of the wall decorations in his room. This afforded an excellent opportunity to expand conversation into the areas of self reflection, perspective taking, inferencing and predicting outcomes. Subsequent topic transition was characterized by strong use of familiar phrases and segue. Chris was very much wanting to discuss aspects of peer relationships once again. Provision of solution based scenarios continues to be an integral component of these discussions, allowing for stronger introspection and decreased emphasis and/or perseveration on highlighted challenges. Speaker- listener reciprocity continues to expand with cycles of exchange consisting of increased length and complexity.

She requested adding literacy goals to POC. Goals will be probed and added in upcoming sessions. O: Noah was first asked to identify which item in a field of 4 belongs in the provided category (e.g. “food” is the category with visuals provided of a baseball, a cookie, a spoon, and a truck). He completed the activity with 85% accuracy. He was then asked to label categories when given 3 items within the category and a field of 3 category answer choices. He completed the activity with 75% accuracy. After, Noah asked mixed “wh” questions about a picture scene. He answered the questions posed with 80% accuracy. After, Noah was asked mixed “wh” (who, what, where) questions about a 1-sentence story when provided with 3 consistent visual answer choices (i.e. same answer choices for each question about the story). He completed the activity with 80% accuracy. He then completed a pronoun labeling activity with moderate support and verbal answer choices, which he completed with 60% accuracy. After, Noah was asked to identify the superlative (e.g. big, bigger, biggest) in a field of 3 visual answer choices. He completed the activity with 70% accuracy. Lastly, he was asked to identify which image displayed “more” and “less” in a field of 2 visual answer choices. He completed the activity with 80% accuracy.
