Occupational Therapy Treatment Session
Lesson Outline
Richard was seen in his room for a 45-minute treatment session. His session focuses on improving his postural control, integration of retained reflexes, visual motor skills and integration, fine motor skills, dexterity, manipulation, ocular motor development and graphomotor development/keyboarding as an alternative means to written communication. Began treatment with alternating between painting a Valentine's Day dog canvas and cursive review. He would paint one part of the dog and then write/review one to two cursive letters. Richard required moderate verbal cueing to correctly form previously learned cursive letters "i, u, w, r sand t". He needed reminders like where to start letters, to dot letter i after connecting and finishing whatever word it is in, and to size letters correctly (like to make the letter t tall). He did well with following directions and not getting frustrated. Good session.

Lesson Outline
Luke was seen in his room for a 45 minute treatment session today. Focus of treatment was on improving fine motor skills (hand strength, grip strength, dexterity and manipulation), visual motor integration, ocular motor skills, and graphomotor skills/keyboarding as an alternate means to written communication. Began treatment at his table with writing activity. We continue to focus on letter formations, line placement, spacing between words and refinement. Continued with visual motor work with sequence drawing activity. Luke was able to recreate by following a step by step drawing sequence the following objects: flowers, a cat, a castle, and a bunny. He us continuing to make his letter "o" the wrong way and requires verbal cues " to make a c first when making an o". He can readily copy a circle, square, traingle and cross. Good session.

Lesson Outline
Luke was seen in his room for a 45 minute occupational therapy treatment session. Focus of treatment was on improving his bilateral integration/hand usage, fine motor development, graphomotor skills, postural control and sensory processing/organization for improved general performance in various environments. OT brought shoe tying book to start teaching Luke this skill. Luke said he wasn't ready for shoe tying so we will try another time. Began session with Simon Says to work on listening skills, crossing midline, and contralateral movements. Luke was able to follow directions well and knew when do a movement and when not to do one. He was unable to do a smooth jumping jack or contralateral stride jumps. He is able to perform homolateral movements well. Sat a table to continue work on CVC valentine days worksheet to work on visual motor and graphomotor development. Luke required cues to make the letter "o" in a counter clockwise direction. Worked on Print Island for upper case letter A and V today. He wrote them well. Had Luke write a sentence today in Print Island workbook. He wrote the letter "j" backwards but overall demonstrated excellent writing.

Began session outside with Luke providing him with deep proprioceptive input through various activities for 15 minutes.Worked on contralateral movements to get the two hemispheres of the brain to communicate during play. When back in his room noticed increased attention to task for longer durations.Worked at his desk to write CVC words with short e practice. For this activity I focused on his formations and line placement of the letters and mixed in some phonetics/reading work. Worked on visual motor control with higher level maze work. When the maze was difficult for Luke he got frustrated which resulted in impulsiveness which in turn made him go off the road/tracks of the maze. OT cued him to slow down, take a deep breath and to go for quality versus speed. Good session.

Continue to work on bilateral hand tasks.

Richard was seen after lunch and recess. Began session working on keyboarding skills via typing.com. Richard is slow and deliberate when typing making sure to use the correct fingers to type the individual keys. His typing speed at this time is about on average 4 to 6 words per minute with great accuracy. Continued with Michigan Tracking Program which works on ocular motor development. Richard was able to scan a jumbled paragraph to find 8 words with one error in 1 minute 22 seconds. For his second scanning attempt he was able to find 8 words in a jumbled paragraph in 48 seconds. Great session.

Began today's treatment with introducing Luke to a "storm starter cloud" activity/worksheet. This activity addresses anger and identification of "storm starters" which are things that make you angry. This helps the child idenify which things/situations bother them and trigger them and what things/situations do not. Luke was somewhat resistive to this activity but we will continue to work on this. Continued with working in I Spy book to find hidden pictures which addresses visual discrimination. Luke was made to work on his graphomotor skills while doing I Spy by having to write each object he needed to find in the hidden picture. He is able to find hidden objects well with minimum assistance provided. He continues to work on letter formations and line placement for print.

Luke was seen in his room for a 45 minute occupational therapy treatment session. Focus of treatment was on improving his bilateral integration/hand usage, fine motor development, graphomotor skills, postural control and sensory processing/organization for improved general performance in various environments. Began treatment playing Monopoly Junior but while playing went over Zones of Regulation and "Triggers". While playing concentrated on following rules, turn taking, expected versus unexpected behaviors. Luke did a better job with following rules today. He demonstrated overall better regulation while playing and did not get upset when other player did well or when he had to pay other player. Continued to focus on whole body listening for increased attention and focus on multiple step directions. Good session and slow and steady gains being observed.

Richard worked on motor planning and core strength with peer outside. Significant improvement noted in his ability to coordinate opposite side of the body movement patterns to a beat. Richard was also able to sustain supine flexion and prone extension for 10 second/2 reps, demonstrating increased overall strength and coordination. Richard was also able to demonstrate good executive functioning skills to plan and execute steps needed to complete a movement game, requiring team mates to work together to complete a multiple step task.

Richard continued to work on fine motor coordination and strengthening. Began with Pencil pushups 10 times with visual and verbal cues for positioning and speed. Noted an improvement in his ability to isolate finger movement from his wrist while shading in areas in a diagonal pattern. Right to left pencil movement patterns continue to be challenge. Ended the session completing 2 sessions in his typing program, focusing on home and top row. Richard continues to work slowly to try and avoid mistakes but is good about trying to use the correct finger for each key.

Reviewed coordination and core strength activities. Richard was easily able to demonstrate 10 jumping jacks, skipping and cross crawl movement patterns. Some difficulty initially to motor plan opposite side arm/leg strides but with verbal cues and practice it became easier for him. He completed supine flexion while kicking the large therapy ball with bilateral feet and prone extension while pushing the ball away with bilateral hands 10x/2 reps each. Minimal difficulty keeping his feet in a elevated positioning while maintaining supine flexion position.
