Richard was seen for his last OT session with this therapist for the school year. He was able to read simple directions with minimal verbal assistance to follow a multiple step cooking recipe. He demonstrated good executive functioning skills, fine motor and bilateral hand skills throughout the session. Richard has demonstrated significant improvement this year, including fine motor strength and coordinating, visual perceptual and motor skills, gross motor coordinating and motor planning, oculomotor skills, sensory processing and overall confidence. Richard continues to struggle with some executive functioning skills and organization. An end of the year progress report will be sent to the school including VMI results and goal progress.
Richard was seen in his room for a 45-minute treatment session. His session focuses on improving his postural control, integration of retained reflexes, visual motor skills and integration, fine motor skills, dexterity, manipulation, ocular motor development and graphomotor development/keyboarding as an alternative means to written communication. Finished standardized testing today using the Bruininks-Oseretsky Test of Motor Proficiency, 2nd Edition. It is intended for use by practitioners and researchers as a discriminative and evaluative measure to characterize motor performance, specifically in the areas of fine manual control, manual coordination, body coordination, and strength and agility. Richard participated well and was able to complete all tasks presented. During the bilateral coordination subtest Richard demonstrated slight difficulty with separating his UE movements from his LE movements and with contralateral movements. Test scores to follow.
Luke seen in his room for a 45-minute treatment session. Luke participated well and was able to complete all tasks presented. Luke was also asked to write a sentence that had every letter of the alphabet. He has a few mistakes which were - not placing the letter "j" under the writing line, making the letter "O" counter clockwise versus clockwise. He needs cues to space better between words. Otherwise his handwriting looks functional/good and he did a great job today with staying on task. Test results to follow.
Assignment
Continue to practice writing sentences over the summer.
Luke seen in his room for a 45-minute treatment session. Continued giving him the standardized testing today using the Bruininks-Oseretsky Test of Motor Proficiency, 2nd Edition. This test is intended for use by practitioners and researchers as a discriminative and evaluative measure to characterize motor performance, specifically in the areas of fine manual control, manual coordination, body coordination, and strength and agility. Luke participated well and was able to complete all tasks presented. Great task compliance reward Luke's effort at tend of session with travel Connect Four game which works on manipulation and dexterity by translating a small object in and out of your palm.
Libby was seen in an office for a 45-minute treatment session with focus of session on improving her fine motor skills especially in the area of grasp patterns, ocular motor skills (saccades), self care skills, handwriting, as well as improving her flexibility for wearing different clothing and wearing her hair different ways. Began session with game to work on manipulation and dexterity with moving/translating small object in and out of hand to and from fingertips. We reviewed all previously learned letters which included capitals "A, B, C, D, E, F, G, H, I, J". Libby was able to successfully identify all except letters J and H today. Went over the next five capitals in the alphabet which included letters "K. L, M, N, and O". She was unable to identify K today but we made sparkle letter index cards which she will be able to take home for practice over the summer. Libby wore shorts today for the first time to school. OT and all the teachers told her how proud they are of her and seeing her wearing different things. Great session.
Assignment
Continue to practice the capital letters for recognition.

Met with Amanda today, Libby's mother, to go over her progress in OT and to discuss Libby's learning style. Discussed how Libby was doing well and making steady progress in 1:1 learning environment versus her small group setting in her preschool classroom for retention and recall of information. Discussed monitoring her progress and revisiting if a neuropsychological testing may be beneficial in the future to hone in on her specific learning style.
Richard was seen in his room for a 45-minute treatment session. His session focused on improving his postural control, integration of retained reflexes, visual motor skills and integration, fine motor skills, dexterity, manipulation, ocular motor development and graphomotor development/keyboarding as an alternative means to written communication. Continued working today on writing in cursive for Mother's Day coupon book. Richard was able to write his name in script; however, relies on OT to talk him through (with auditory clues from Script Island) writing the letters. We continued working on refinement of cursive and Richard was taught high connectors today.

His session focuses on improving his postural control, integration of retained reflexes, visual motor skills and integration, fine motor skills, dexterity, manipulation, ocular motor development and graphomotor development/keyboarding as an alternative means to written communication. Began today's session with cursive handwriting practice. Since Richard has learned every letter of the cursive alphabet he was asked to write words by connecting letters today to make his mom a Mother's Day coupon book. When provided with visual and verbal cues Richard was able to write in cursive slowly and methodically. This style of writing will take practice before it becomes functional but his end result was beautiful and he was proud of his writing. He continues to require specific cues to loop his letter like cursive "l and e" backwards but overall seems to grasp the movements involved in writing in script. Great session.
