Luke seen in room for 45-minute treatment session. Focus of treatment on improving and strengthening his emotional intelligence/self-regulation, improved attention/participation in both preferred and non-preferred task, bilateral coordination, refinement of visual motor skills/graphomotor development, fine motor precision, and sensory processing. Began session with "How do you feel today?" check in worksheet. Luke said he felt happy. He continues to have a runny nose. Needs assistance and reminders to manage his runny nose. Continued treatment with graphomotor development by writing words to describe a picture. Today Luke asked to write on his dry erase board. He continued fine motor precision practice with a maze worksheet and required reminders to stay within the lines and not to take short cuts when solving the maze today. Lastly, worked outside on bilateral coordination skills. Good session.
Assignment
Continue to work on performing skills like jumping jacks and assymmetrical stride jumps at home
Began treatment with a "How Do I Feel Today?" check in worksheet. Luke has been feeling happy and today he felt happy again. Luke did appear to have a stuffy/runny nose during today's session. Continued therapy in his room working on whole body listening and following multiple step directions to improve auditory processing abilities. Luke was given one direction at at time and was able to correctly complete 3/4 directions. Continued session working on fine motor precision skills and had Luke participate in maze worksheets. Lastly, worked on graphomotor skills. Luke was able to write six words to describe a picture. He mixed some uppercase letters with lowercase letter when writing and needed to be reminded to write using lowercase letters. Today, OT brought a sensory seat for him to try. He would also benefit from a visual timer in his room to help focus his attention on non-preferred, teacher-directed tasks.
Assignment
Continue to practice writing words and short sentences independently
Luke seen in room for 45-minute treatment session. Focus of treatment on improving and strengthening his emotional intelligence/self-regulation, improved attention/participation in both preferred and non-preferred task, bilateral coordination, refinement of visual motor skills/graphomotor development, fine motor precision, and sensory processing. OT came to session with a printed schedule with boxes to check as tasks were completed in an attempt to gain increased task compliance. Started session and will continue to start sessions "How Do I Feel Today? Check In. He was asked to circle the word that best describes how you feel. Luke answered "happy". He was then asked to explain why he was feeling this way. He said "I am just happy". Continued session outside with an obstacle course involving movements that require bilateral coordination as as other gross motor play. Luke refused to do jumping jacks today in obstacle and wanted to choose exactly the activities to get from beginning to end of the course. His design involved a lot of jumping and homolateral movements versus contralateral and bilatereal movements. OT will continue to encourage bilateral and contralateral play during future sessions. Lastly, worked at table on graspmotor and visual motor skills. Good session.
Luke was seen for a 45-minute Occupational Therapy treatment session in his room today. Focus of treatment was on establishing rapport since haven't seen one another for a couple months, strengthening his emotional intelligence/self-regulation, improving his attention/participation in both preferred and non-preferred tasks, improving bilateral coordination, refining visual motor skills/graphomotor development, fine motor precision and sensory processing. Luke seemed happy today and seemed glad to be back in school. He wanted to sit on OT's lap during tabletop activities and needed to be repeatedly redirected to sit in his own chair. When OT was reintroducing the Zones of Regulation Luke said "Not this again". He reviewed some previously learned letters today and was able to form 4/5 of the letters correctly. He did reverse the letter "d" as a "b". He wrote a note to a peer across the hall to further practice graphomotor development. Good to be back working with Luke.
School Field Day. Richard's OT end of the year progress report was scored and written up. Please refer to report for scores, progress and recommendations. The report was emailed to Jordy.
Richard was seen for his last 45-minute treatment session today. We worked on transition to summer discussion and played some fun games. Please refer to Richard's end of the school year progress update for details of progression and standardized test scores.
Assignment
Please practice cursive handwriting over the summer
Luke was seen for his last OT 45 minute session on the last school day. Focus of treatment today was on improving bilateral coordination, self regulation and transition to summer. Luke talked about emotion and zones of regulation with his OT today. He continues to be able to appropriately label his feeling and emotions and place them in the correct ZONE. Outside we worked on various activities to improve whole body cross lateral movements. Please refer to end of the year progress update for standardized testing scores and overall progress. It was a pleasure working withe Luke this school year.
Richard worked on visual perception and visual motor activities completing age appropriate mazes and copying geometric shapes drawn on grid paper. 2 out or 4 shapes he required minimal verbal cues to correctly draw the image. He ended the session completing 10 minutes of keyboarding skills with good positioning and finger isolation to locate the correct keys, using visual and auditory cues from computer and looking at his keyboard to find the keys.
Richard continued to work on executive functioning skills with the use of a timer to clean and organize his belongings and pick up trash/scrap paper in his room. The timer was stopped anytime he stopped working then restarted again when he returned to work. Short time segments such as 1-2 minutes appear to be most helpful to him at this time. Richard continues to demonstrate good finger positioning and correct finger use on keys with minimal verbal cues and increased time during keyboarding. He is currently working on Reach out letters O, B, and A with 86% average accuracy and 8 WPM.
Richard began his session with a fine motor warm up of pencil aerobics (pencil pushups and inchworm) 10 times each. He completed mazes to work on visuals perception/motor skills with good accuracy. He continues to present with limited thumb DIP mobility which can result in thumb fatigue and pain when writing for long periods of time. It is recommended that he continues to use an adaptive pencil grip for correct positioning when writing.
