Luke was seen in his room for a 45 minute occupational therapy treatment session. Focus of treatment was on improving his bilateral integration/hand usage, fine motor development, graphomotor skills, postural control and sensory processing/organization for improved general performance in various environments. Began treatment outside to perform exercises to work on bilateral coordination, crossing midline, midline control and trunk strengthening. He participated in activity that had a marble going through an infinity eight maze. Luke had trouble keeping the marble going in maze without having it fall out but did not get frustrated or upset. Continue with self regulation work and discussed what expected and unexpected behaviors were to reinforce concepts. Today Luke was able to freely express to the OT a time when he was in the red zone earlier in the day. This is great progress and he now can begin to work on regulatory strategies for when he is in this zone.

Luke expressed that he was in the red zone during recess when Matthew pulled his flag. This was an appropriate labeling of his emotions.

Began treatment session working on self regulation skills by playing Zones of Regulation Bingo. This activity continues to help with identification of facial expressions and how someone is feeling. Also classification of these feeling into their specific ZONE. Luke seems to be understanding the concepts in ZOR and enjoys working on these skills when they are in a game format such as this. He is beginning to express specifics about his past emotions and is becoming better able to use the zones language and classification system correctly. Continued with visual motor work via writing using Print Island. Luke is now being instructed on the upper case alphabet and is working on the letter uppercase "D". He continues to need cues to stay on task and to use whole body listening skills.

Richard was seen in his room for a 45 minute treatment session today. Focus of treatment was on improving fine motor skills (hand strength, grip strength, dexterity and manipulation), visual motor integration, ocular motor skills, and graphomotor skills/keyboarding as an alternate means to written communication. Began today's session with slime sensory hand play. Continued with practicing manuscript writing to complete a Thanksgiving activity. Richard needs cues to use pencil with grip and demonstrated a much more efficient grasp pattern when using his grip. When writing, Richard takes his time and thinks to form each letter correctly. His motor memory for correct formations is not automatic at this time but he is aware of the correct formations. He continues to require cues to make the printed letters "a and n". His writing is getting neater and smaller!
Good session!
Richard has new glasses and wore them throughout the session. Began session working on fine motor skills while drawing on blank scratch art paper. Minimal verbal and tactile cues were needed to correctly hold the scratch stick with a grip. Demonstrated good visual perceptual skills coping 4 3-D images with the Kanoodle game. Richard completed 3 lessons on the typing.com program with good accuracy and attention to using correct fingers for each top row key.
Richard demonstrated excellent participation, attention and good self regulation throughout the session. He worked on fine motor and visual perceptual skills while following step by step visual instructions to put together a toy helicopter. He did well following the steps and locating each part but moderate physical assistance was needed to use small screwdriver and wrench together, a task requiring the use of bilateral integration and fine motor skills at the same time. Richard was very happy with himself when he completed the project.

Began treatment with mat exercises to work on reflex integration for the retained ATNR that Richard continues to have. Tested TLR reflex which seems still be be active with look back not forward. Continue with desk work and cursive instruction. Today Richard was instructed on the lower case letter "o" in Script Island. This is the first high connecting letter Richard has learned. He was able to write it well after being provided with visual, auditory and kinesthetic cues. Lastly had Richard participate in Ann Arbor Tracking program in which he had to track six words of a sentence in a jumbled paragraph. He was able to find the six words within 40 seconds. His ocular motor abilities of pursuits is improving and he is gaining speed and accuracy for tracking. Great session.

Luke had a great session today and was focused and cooperative. Began session with a Turkey/Thanksgiving activity in which he had to write three things he was thankful for and then color and cut out his turkey. This turkey was then made into a hat. Luke was able to independently write the three things he was thankful for which were: Mom, Dad and his Dogs (he says Charlie is his favorite). Continued with Zones of Regulation work in which Luke was asked to identify how someone was feeling based upon their facial expression. He was successful in facial recognition and matching the zone to the expression. Reviewed expected and unexpected behaviors and had Luke express one thing that makes him feel in the yellow or red zone. He said when his sister comes into his room without permission (without asking or knocking). Lastly, played Guess Who game with peer. Great session.

Luke had a runny nose during today's session. He does not like to use tissues which maybe a sensory related issue. During today's session Luke worked on writing CVC words to practice writing in manuscript. He has now completed the entire lowercase portion of Print Island but needs continued practice to reinforce the correct motor memories for letter formations. He also would benefit from continued auditory cues for line placement/size of these letters to refine his writing. He required verbal cues to form the letter dm b an and o correctly today. We continue to work on self-calming strategies like asking for a break when starting to feel frustrated or annoyed. Luke seems a little under the weather with a runny nose this week.

During today session we worked on Zones of Regulation with teaching expected versus unexpected behaviors with a game that involved getting points for naming if a said behavior was "expected" (appropriate) versus "unexpected" (inappropriate, makes others feel uncomfortable). Luke was able to label appropriately what was expected versus unexpected behaviors with 100 percent accuracy. Continued working on letter formation, sizing, and line placement via Print Island. Luke was taught the lowercase letter "z". He was able to form this letter adequately.

Dr. Jordy joined our game with expected versus unexpected behaviors. Luke had a difficult time today during his session. He called me a "freak" and yelled things like "I don't need a tissue!". He has a runny nose and was wiping it on his shirt. He was disrespectful today and raised his voice a lot. He was going to run away from OT after going to the bathroom but changed his mind when he saw Dr. Jordy or Megan in the hallway.

Participated in a letter activity and was given boxes to help with sizing and letters. Continued session with Zones of Regulation work. Went over Zones and Luke seems to understand each zone well. Taught Luke one strategy for self calming when feeling frustrated (yellow zone) or angry (red zone). Calming strategy was pretending to hold a hot chocolate - smell it and then blow on it to cool it down. Luke seemed to like this strategy today and demonstrated it to Mom when she arrived to pick him up. Good session.
