Libby was seen in her room for a 45-minute treatment session with the focus of therapy on fine motor skills specifically grasp patterns, ocular motor skills (saccades), self care skills, and handwriting. OT has also focused on sensory processing and having Libby be more comfortable with wearing a variety of clothes (weather appropriate) and different hairstyles (only wants to wear her hair one way which is down). Libby continues to be encouraged with a sticker chart to wear different clothes and her hair different ways. She is excited to report what she wore and how she styled her hair over the past week to earn stars on her chart. We continued therapy with recall for upper case letters. Libby is now able to identify all of the letters of the upper case alphabet with 100% accuracy. We continued our work on Print Island. Libby was taught the "fall down, climb up letter" lowercase "r" two weeks ago. Today she was instructed on the lowercase letter "n". She was able to form and size it well when provided with a coconut and a trunk as a visual boundary. By the end of the practice page she was able to form it independently well. We continue to work on hand strength, dexterity and manipulation to work the intrinsic muscles of her hands. Great session, Libby is making good gains this year.
Libby was seen in her room for a 45-minute treatment session with the focus of therapy on fine motor skills specifically grasp patterns, ocular motor skills (saccades), self care skills, and handwriting. Libby participated well and was focused throughout her treatment session. Continued to work on increasing dexterity, manipulation and hand strength with putty and hidden object activity. Libby enjoyed using the intrinsic muscles of her hands to find the hidden objects in the moderately resistive putty. We continued therapy with visual recall and recognition of both uppercase and lowercase alphabet letters today. Libby was able to recognize and name every letter of the uppercase alphabet except for the letters V, P and J. Last week she needed minimum verbal cues for the letter "B" D" this week she knew them. Last week she couldn't recognize H, v and O this week she knew H and O. Both weeks she could not recognize uppercase "V". She is making solid improvement with recall for her letters. We will start with handwriting program next week 10/03/2024 using Print Island to teach Libby the correct formation of the lowercase letters. She needs to work on her recall and recognition of the lowercase alphabet as well. Good session, progress is steady.
Libby was seen in her room for a 45-minute treatment session with the focus of therapy on fine motor skills specifically grasp patterns, ocular motor skills (saccades), self care skills, and handwriting. Libby participated well and was focused throughout her treatment session. Work on refinement of fine motor precision and manipulation skills with activity involving using tweezers, spray bottles and small beads. She completed this activity and followed directions well with her brother Teddy. We continued with alphabet recognition for both the upper and lower case alphabet today. She was almost able to identify all the letters of the uppercase alphabet today she got 23/26 correct. She is able to identify about 1/2 to 3/4's of the lowercase alphabet independently. She is improving with wearing her hair in different ways and not limiting the clothes she will wear. She is making steady gains.
Libby was seen in her room for a 45-minute treatment session with the focus of therapy on fine motor skills specifically grasp patterns, ocular motor skills (saccades), self care skills, and handwriting. Started session with hanging star chart and alphabet in Libby's room. Libby communicated with OT that she wore shorts and her hair up when playing soccer. She received stars on her chart for both. Continued treatment with having her dig through moderate resistive putty with her hands and fingers to find hidden objects within the resistive putty. She was able to find all hidden objects independently.
Libby was able to recognize 23/26 letters of the upper case alphabet today. She needed verbal cues to recognize M and V and was unable to recognize F, N and S. Continued treatment working on manipulation and dexterity skills to make the letter "F" using pegs on Lite Brite board. Lastly, worked on ocular motor skill of "convergence" to make nose on lady bug hung on door. Great session and steady gains seen.
Assignment
Continue to practice recognition of alphabet in everyday play
Luke seen in room for 45-minute treatment session. Focus of treatment on improving and strengthening his emotional intelligence/self-regulation, improved attention/participation in both preferred and non-preferred task, bilateral coordination, refinement of visual motor skills/graphomotor development, fine motor precision, and sensory processing. Talked to Luke about his new school trial and went over acceptable interactions in bigger classroom setting. We played a game "what would you do? -for example - "you want to ask you teacher something" do you raise your hand or blurt out your thoughts? If a classmate takes your pencil what do you do? If someone steps on your foot in line what would you do? For the most part Luke was able to understand the "appropriate" behavior for a traditional classroom setting. He needs to be reminded when things don't go how he likes with a peer to get a trusted adult to help instead of trying to handle it himself. Continued treatment with participating multiple step activity that had Luke follow multiple step directions. He required min to mod assist. Continued with maze worksheet to work on his fine motor control and precision. Lastly, participated in a "magic c" writing worksheet to help Luke with correcting his formation of the letter "o" which he tends to make in a clockwise direction versus counter clockwise. He makes all other of the "magic c" letters correctly. He continues to make steady gains this year and his regulation and participation has shown much improvement.
Began treatment in Luke's room to work on self regulation strategies. OT and Luke went over each emotion that was represented in "Inside Out 2". Luke was asked to tell a time when he experienced each emotion. The emotions discussed were "boredom", "envy", "angry", "sadness", "joy/happiness", and "anxiety". Some examples of Luke's answers were: anger- whenever my sister does things I don't like, joy/happiness: when I went up to the farm and I got cars. He did a great job talking about emotions and recognizing his emotions. Continued with visual motor integration with a maze worksheet. Luke was observed to hold his head up with his left hand. He required verbal cues to find his way through the maze, stabilize the paper he was working on and use careful movements to stay within the lines of the maze.
Libby was seen in her room for a 45-minute treatment session with the focus of therapy on fine motor skills specially grasp patterns, ocular motor skills (saccades), self care skills, and handwriting. Began today session reestablishing rapport which Libby which was easy to do. Libby was excited to share what she wore over the summer and was interested in putting back up her reward chart for her hair and clothes. She further reported that she only put on her braids when she was in camp otherwise she wore it down. Continued treatment with ocular motor activity that worked on convergence. Libby was asked to draw a small nose on ladybug faces hung on vertical plane. She continued to work on depth perception with straw and small stick activity. She did well with both tasks. Lastly, we went over alphabet to determine which uppercase letter she was able to identify from recall. She was able to recall 17/26 letters correctly. She was unable to identify the following letters: F, E, H, J, M, O, S, U, and V". Great first session.
Assignment
Continue to practice identification fo the alphabet.
Began session at desk and began with daily check in worksheet "how do you feel today". Luke reported with he is having a "yellow" day. This refers to "joy" from "Inside Out 2". Continued to talk about anger and appropriate ways to handle this particular feeling. Luke is more receptive to discussing emotions. Continued treatment with maze activity. Luke required moderate to maximum assistance to follow symbol to find his way through maze. He knew his right and left and was able to follow directions well today in this one to one setting. Lastly, worked outside with activities to reinforce bilateral and assymmetrical movements. Luke was able to perform 5 jumping jacks in a row when provided with verbal cues of "star, solider" and demo of movement. He continues to have his arms and legs not coordinated without cues and demo, but with them he is successful. Good session.
Assignment
Continue to practice bilateral activities like jumping jacks, jumping rope, catching with two hands, swimming

Lesson Comments
He told me that he went to Jupiter Christian Academy yesterday. When we talked about anger, he told me that he was angry at his mom when she wouldn't text his friend Lucas mom for him to set up a play date.
Luke seen in room for 45-minute treatment session. Focus of treatment on improving and strengthening his emotional intelligence/self-regulation, improved attention/participation in both preferred and non-preferred task, bilateral coordination, refinement of visual motor skills/graphomotor development, fine motor precision, and sensory processing.Began session with "How Do I Feel Today/" check in worksheet. Again, Luke said good and happy. He has been having improved self regulatory behavior this school year and seems happier. Today he was interested in talking about emotion when OT brought out an Inside Out character worksheet. Luke saw Inside Out 2 over the summer and was talking about the new emotions that were in the movie. Luke seems more accepting about talking about emotions this school year which is going to make working on self-regulations so much more effective. The emotions in Inside Out 2 that we discussed today were Anxiety, Boredom, Embarrassment, Envy, Nostalgia, Joy, Disgust, Anger, and Sadness. We worked on fine motor precision through a maze worksheet and bilateral coordination with an obstacle course outside. Good session.
